BRIGHT (light) Berkshire West Responsive Integrated GP and Home-Visiting Team

Berkshire West Responsive Integrated GP and Home-Visiting Team (BRIGHT (lite)) Service, is providing active, proactive and reactive home-based clinical assessment across Berkshire West, supporting same-day acute and pre-booked and planned home visits.

BRIGHT (lite) provides additional senior clinical decision-making capacity, operating as an adjunct to existing and established neighbourhood structures, including, primary care, community services, social care and voluntary sector. The service is designed to be flexible and collaborative, offering easy access for referrers while expecting timely, clear communication between primary care and community providers.

The service aims to make best use of already established systems (as additional capacity and not duplication) and minimise onward conveyance by supporting patients to remain safely at home, wherever clinically appropriate.
